Benefits Summary
G.A.S. has a 125 Plan in place that allows you to withhold Health, Dental, and Life Insurance premiums before taxes are taken out. This lowers your taxable income and reduces your social security and federal withholdings.
Major Medical and Hospitalization is
offered through HealthMarket Consumer Driven Health Plans.
HealthMarket Consumer Driven Health Plans The new plan is designed to put you in control of your health care. You will be able to see any doctor from any specialty without pre-certification. This plan involves the highest deductible offered by HealthMarket; Plan C with a $1,000 annual deductible and Plan D with $2,000 annual deductible. Both plans include HealthMarket StartWell Accounts and "Smart Funds".
HealthMarket StartWell Account is funded through premiums. This provides first dollar coverage with no deductible or coinsurance, (up to an annual account limit of: Individual $1,000; Family $2,000 per plan year). The account is used for the care people seek the most, such as Allergy Tests, Mammograms, Office Visits, and Cardiac Stress Tests. There is an annual rollover feature that transfers unused benefit dollars into the next plan year. This rollover feature provides members with an incentive to purchase healthcare services wisely, since they can use the account to offset future expenses.
HealthMarket Smartfunds are individualized spending accounts for specific medical conditions and procedures. They work like the StartWell Account by providing first dollar coverage, subject to the Maximum Allowable Charge(MAC)
Your HealthMarket plan has a Prescription drug benefit program. The following are the copayments for your prescriptions: $10/$25/$40 retail, $20/$50/$70 mail order through Merck-Medco.
Also included is the Spectera Vision Plan with any of the two HealthMarket plans. As a member, a single $10 co-payment will get you your eye exam and vision prescription. Spectera providers include Eyemasters, United Optical and Wal-Mart. The website for Spectera is www.spectera.com
G.A.S. offers a choice of two Dental Plans through Fortis Benefits.
The PINNACLE PLAN provides dental services through its carefully selected network of private practice dentists and specialists. There are no claim forms, no deductibles, no annual benefit maximum, and no waiting period.
The FREEDOM PREFERRED is a
traditional indemnity dental plan that allows you to seek services at any licensed dentist and submit claim forms for reimbursement. There is an annual deductible of $50.00. The maximum benefit is $1000.00.
Additional Term Life Insurance and Accidental Death & Dismemberment for employee, spouse and dependent children are offered through Unum. Because the plan gives you the benefit of group rates, the prices are better than you could get as an individual. And unlike many group plans, you can choose your coverage amount based on your personal needs: up to $500,000 for employee, $250,000 for spouse, and $10,000 for each child. With Unum's plan, you have the flexibility to choose either Life or AD&D coverage separately, or to choose both coverages.
Long Term Disability is offered through Unum. LTD insurance provides 24-hour protection against the loss of your income if there is a disability that prevents you from working and earning an income. If you meet the definition of disability, you would be eligible to receive a monthly benefit equal to 60% of your basic monthly earnings. Benefits would begin after 90 consecutive days of disability.
COLONIAL Short Term Disability is now offered to all employees with more than one year of service. If you are new to G.A.S. Unlimited, Inc, you will be eligible to participate after your one year anniversary, during the Open Enrollment period. This plan will pay your bills if you were hurt in an off-the job accident or become seriously ill and could not work for a prolonged period of time. You can continue your plan at the same cost should you change jobs or retire.
Other income protecting benefits through COLONIAL
are:
- Cancer Insurance (Catastrophic Illness Coverage)
- Medical Bridge (Medical Plan Deductible Protection)
- Accident Plan (24-hour Family Accident Protection)
The Flexible Spending Account (Cafeteria Plan) is a voluntary plan that allows you to pay for certain expenses before taxes. Expenses include insurance premiums not sponsored by G.A.S., health care costs not covered by insurance and child or dependent care. By reducing your taxable income, both social security and federal withholding taxes will be reduced producing a net increase in take home pay.
The 401(k) Retirement Plan is through Putnam Investments which was ranked #1 Full Service Retirement Plan Provider and #1 for Investment Management Services by DALBAR Surveys, Inc.. Participation is offered to employees after 30 days of employment. As a participant, you may choose from six funds which offer the opportunity for portfolio diversification and you may invest as much as 100% of your income up to the federal maximum of $12,000 per year (2003). Putnam offers a wide range of services, which include:
- Fund prospectuses, quarterly performance updates, quarterly fund updates and annual reports.
- A toll-free telephone service on any business day between 9:00am and 7:00pm EST which allows you to speak with a Putnam representative. You may initiate transfers to existing account balances, receive share-price data, performance information and statistics. Account balances are available toll-free 24 hours a day.
- Loan privileges.
- Rollovers from other retirement plans.
"Catch-up Contributions for Participants Aged 50 and Older:"
The G.A.S. Unlimted, Inc. 401(k) Plan will also allow for pre-tax/elective deferral contributions in excess of legal or plan limits under Code section 414(v) (i.e., "catch-up" contributions). The additional "Catch-up" Contribution for the plan year 2003 is $2,000.
G.A.S. Unlimted, Inc. is
pleased to offer a new benefit to you. Effective July 15, 2002, you will
have the opportunity to participate in a 529 Education Plan through Putnam
College Advantage. As education costs continue to increase, saving for
college expenses ha now been made easier. You can use your savings at any
accredited college in the country. And you also get a great tax
benefit.
As an employee of G.A.S. Unlimted, Inc. you are eligible
to join Houston Federal Credit Union. Membership is also extended to your
family. And once you are a member, you can retain membership even if you
later move or change your current employment. To join just fill out an
application and make an initial deposit of $50.00 or more. These are no
membership fees. Deposits may be made to your account by payroll
